To elaborate on Flans' offered quote on this song being about "mania and panic" - an 'outside voice' is the voice that kids are told not to use inside (instead, their 'inside voices') because it is (according to Farlex Dictionary of Idioms) "A wild, loud, or uncontrolled voice; a voice that is not appropriate for an indoor environment."
